Please complete forms found under the forms tab, new students need to complete the ODJFS Enrollment form, returning students will need to update theirs at orientation. Please also fill out our enrollment form. New students and old friends with summer birthdays will also need to have a physical filled out. Remember to include the name, address, and phone number of your child’s doctor and dentist. Please bring all completed forms to orientation. All children must have the completed forms on file before school begins as required by the State of Ohio.
Under the forms tab you will also find a Lunch Bunch form. Please fill it out and send it back to me as soon as possible. We would love to have an idea of the size of these groups before school starts so that we are able to have them staffed. Thank you for your cooperation.
We hope you are enjoying your summer. If there are any changes in your plans, please call us immediately. We are looking forward to fall and another successful school year with your children.
